The top and bottom of a printed form have been separated and joined by 2 blank leaves, all pasted together to form a long document. The form has been filled in, signed and witnessed. There are some corrections, but it seems to be a finished document. It has an embossed paper seal in the upper left corner. There are several annotations on the verso.
